PERMISSION FOR THE ADMINISTRATION OF PRESCRIPTION MEDICINES

If your child needs to have a dose of prescribed medication during the school day please ensure a signed form accompanies the medicine, which should be delivered to Matron in the School Office upon arrival at school. Medicines cannot be given without this written permission.
